ADMISSIONS

Completed applications should be brought to our main office at 900 Madison Avenue. Our office is open Monday - Friday, between 8am and 3pm. In addition to the application below, each parent is required to submit:
  • Copy of full immunization records from a physician or school nurse
  • Two previous years of academic records with standardized test results
  • Disciplinary records (if applicable)
Upon receipt of the information above and completed scholastic testing, you will be notified of admittance by the school office.
